Description
Cleft Palate Speech/ Velopharyngeal Dysfunction (VPD) is complex and multifaceted. Even following primary repair of the palate, the child may present with cleft palate speech, and a proportion of children will continue to have VPD. Accurate and reliable assessment by the Speech Therapist is important for differential diagnosis and direction of management. With Submucous Cleft of the Palate, the Speech Therapist is often the first professional the child is referred to. Knowledge, assessment, and profiling of cleft palate speech and/or VPD, is therefore essential.
Perceptual assessment of cleft palate speech and/or VPD remains at the core of speech methodology, and is the gold standard in providing outcome data for clinical assessment and research to support evidence-based practice in the field. Although there is yet no single internationally standardized approach, there is a range of specified and internationally agreed upon articulation and non-articulation parameters in cleft palate speech and VPD, as well as guidelines on speech sampling and elicitation methods. Accurate and valid assessment of the full range of possible speech outcomes requires a clinical tool that encompasses a systematic framework and profiling system.
In this seminar, participants will be introduced to the range of speech outcomes in the assessment of cleft palate speech and/or VPD dysfunction and will be orientated to the Cantonese-Cleft Speech Assessment Tool (C-CSAT, Yiu et al., 2019), which has already been disseminated to over 200 Speech Therapists in Hong Kong and Macau. There will also be opportunities for listening and ratings practice and clinical phonetic transcription In Cantonese, based on audio and audio-video recordings of real cases.
Outline
AM Session
Meet and listen to 2 surgeons who will talk about “Cleft Palate +/- Lip and Primary Surgical Repair” and “Velopharyngeal Dysfunction and Secondary/Velopharyngeal Surgery”.
A framework for profiling and analysing Cleft Palate Speech/VPD will be described with examples. Updates will also be provided on terminologies and classification of cleft palate-related speech disorder(s) and VPD.
Learn about the development and item construction of the C-CSAT. Emphasis will be provided on the importance and types of speech sampling materials (including rationale for phonetic environment) and measurement methods.
PM Session
In the afternoon session, learn about the standardized listening and ratings protocols used in the C-CSAT. As cleft palate speech/ VPD is so complex and multifaceted, a listening and ratings protocol that outlines which speech outcome is to be rated based on which speech sample, is essential to the validity of the assessment findings.
Practice clinical phonetic transcription, listening and ratings using the C-CSAT charting and rating forms. Audio and/or audio-video recordings of real cases will be used.

Biography of Speakers
Dr. TSUI Siu Yan, Bess is a dedicated pediatric surgeon with expertise in various areas of pediatric surgery. Graduating from the Chinese University of Hong Kong in 2008 and obtaining her Fellowship in Paediatric Surgery in 2015, she has acquired a strong foundation in this specialized field. Bess's dedication to advancing pediatric surgery extends beyond her clinical practice that she holds the position of Honorary Clinical Assistant Professor at the Chinese University of Hong Kong, where she contributes to the education and training of future healthcare professionals in the field of pediatric surgery.
Bess has a special interest in pediatric gastrointestinal surgery and cleft surgery. She actively participates as a member of the Cleft Team, collaborating to review and enhance the quality of cleft care and surgical outcomes. Bess also enjoys to share her experience and has been the guest lecturer of the Professional Diploma Programme in Advanced Nursing Practice, CUHK Nethersole School of Nursing, invited lecturer for Post-Registration Certificate Course (PRCC), and invited speaker for The Association of Hong Kong Operating Nurses (HKORN) on Pediatric Surgery and Perioperative Nursing Care.
Dr. CHOI Wing Shan, Winnie completed BDS, MDS and PhD at the Faculty of Dentistry, the University of Hong Kong. She has been a specialist in Oral & Maxillofacial Surgery (OMFS) in Hong Kong since 2010 and is one of the first locally trained female OMF surgeons in Hong Kong. She practises a wide scope of Maxillofacial Surgery, including Oral oncology and microvascular reconstruction, orthognathic surgery and obstructive sleep apnea, cleft and craniofacial syndromes and orofacial pain. She appears frequently in regional and international conferences and courses as invited speaker. She has also published over 50 peer-reviewed journal articles and conference abstracts.
As the Clinical Assistant professor in OMFS at the Faculty of Dentistry, the University of Hong Kong in 2011-2023, apart from clinical teaching and research, she had served as the Assistant Dean in Clinical Affairs, Undergraduate and Postgraduate Programme Directors in OMFS. She also contributes to the profession and has served as the Chief examiner for the OMFS specialty board exam of the College of Dental Surgeons of Hong Kong (CDSHK), an examiner for the Dental Council of Hong Kong and the OMFS specialty board member of CDSHK.
Ms. YIU Oi Yan, Yoyo is a paediatric speech therapist with significant experience in managing communication and swallowing problems in children. With a master's degree in Paediatric and Adolescent Medicine from HKU and training at renowned international medical centres, including the Children's Hospital of Philadelphia and Chang Gung Memorial Hospital, she has developed expertise in this specialized field.
Miss Yiu's clinical focus includes the field of cleft lip and palate, where she actively enhances assessment tools for outcome measures and fosters collaboration with surgeons in clinical practice. Her dedication to research is evident through her publications in peer-reviewed journals and presentations at scientific conferences.
Alongside her clinical and research work, Miss Yiu is passionate about professional training. In addition to conducting teaching sessions for speech therapists and students, she provides inter-professional training to professionals in related fields such as radiology, dentistry, and nursing, expanding her commitment to knowledge sharing and collaboration across various disciplines.
Ms. CHAN Ching Shun, Sabina has been serving public hospitals as a Speech Therapist for over 25 years. She received a Master’s degree in Applied Linguistics and a second master’s degree in Clinical Gerontology after having the qualification of being a Speech Therapist. She was previously the team head of the Speech Therapy Unit of the Hong Kong Children’s Hospital (HKCH) and participated in developing speech therapy services there from commissioning phase to the opening of service. She worked with her team to provide in and outpatient service for paediatrics including cleft when she worked in HKCH. She has the passion to have a standardized reporting and documentation system for cleft palate speech and velopharyngeal dysfunction and thus participated in developing the C-CSAT.
Ms. CHOI Kai Yan, Cello has a Bachelor degree from the Department of Linguistics and Modern Languages of CUHK and became a speech therapist after completing the Master of Speech Therapy at the Hong Kong Polytechnic University. Cello’s journey into the world of cleft speech began during her master studies, where she had the privilege of working in Prof. Pereira’s cleft clinic. Her role in the working group and development of the C-CSAT was around her academic background in phonology and phonetics and her clinical speech therapy experiences in the area of cleft lip and palate.
Ms. Choi is currently a speech therapist working with cleft children. One of the most rewarding aspects of her work is witnessing the transformation that could occur in her patients with differential diagnosis and cleft speech articulation therapy. Whether it is seeing how a child’s speech improve after a recommended surgery or helping a child say a correct sound, the joy of seeing her patients succeed is unparalleled. Apart from cleft, she also has special interests towards pediatric dysphagia.
In addition to her clinical work, Ms. Choi is also keen on doing education within the field of speech therapy, sharing knowledge and expertise with fellow speech therapists and professionals of other disciplines in different settings.
Prof. PEREIRA Valerie Jane is Assistant Professor and Speech Therapist with the Department of Otorhinolaryngology, Head and Neck Surgery, CUHK. She is the current Clinical Director of the MSc in SLP program and Program Leader of the Professional Diploma in Communication Sciences and Disorders. She has over 20 years of clinical and research experience in craniofacial anomalies. She was Specialist Speech Therapist with the Superregional Craniofacial Service at Great Ormond Street Hospital (London UK) and subsequently worked with the North Thames Regional Cleft Service (UK) for over 10 years. She is the current Chair of the Cleft Palate and Craniofacial Conditions committee of the IALP International Association of Communication Sciences and Disorders, a worldwide non-profit ortganization, and part of the 14-member Global Speech Task Force, International Confederation of Cleft Lip and Palate and Related Anomalies. She is the IALP representative for the Transforming Faces (Canadian cleft care charity)-IALP Mentoring programme, which serves to provide support and mentoring to Speech Therapists in underserved and unserved geographical regions.
